Overview
In 1971, Yale University established Yale Health to provide health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Yale Health has more than 50,000 members including students, staff, faculty and their families who come from every state in the country and almost every country in the world. With over 50 years of service to the Yale community, our state-of-the-art facility at 55 Lock Street, is where our members receive most of their care from our 150+ providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15-bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x-ray and ultrasound and a full-service retail pharmacy.
The Ambulatory Clinical Pharmacist works collaboratively with clinicians, pharmacists and support staff in accordance with an established collaborative practice agreement to provide seamless, quality care for patients at Yale Health. As an ambulatory clinical pharmacist with a focus on metabolic health you will educate patients on self-management by developing the patient’s knowledge, skills, and abilities that are necessary to manage their condition and improve their health outcomes. Potential for future focus on managing other chronic diseases.
Responsibilities include
Primary:
Maintains competency skills by participating in continued education and professional development.
Works with a multifaceted team using their individual disciplinary expertise to collaborate on providing comprehensive age-appropriate nutrition, medication, and lifestyle management education.
Recommend, adjust, and implement therapies that require critical thinking and independent judgment in collaboration with the patient, referring physician, medical director, and other designated members of the health care team.
Provides patient management by assessing, planning and documenting.
Implements interventions as per department guidelines, management protocols, and specific patient needs.
Evaluates patient's progress and revises plan of care as needed.
Maintains extensive follow-up for complex patient population while exploring pertinent resources for improved outcomes.
Demonstrates proficiency in all aspects of patient education.
Secondary:
Assists in the development of educational materials.
Assists in the development of P&T presentations.
Attends annual conferences (American Dietetic Association, The Obesity Society, AACE, etc.) for continued education and training to related disease states.
Assist in the training and mentoring of pharmacy students or physician assistant students.
Demonstrates the ability to make decisions, take appropriate action and follow tasks through to completion.
Responds cooperatively to managers and staff members in other departments to promote teamwork.
Participate in quality initiatives; including functions that support audits, reporting, evaluation of quality opportunities and concerns, and creation and implementation of corrective actions as requested.
Consult with clinicians on dosage, medication interactions and other clinical issues.
Counsel patients on correct usage of drugs and on possible drug interactions with other prescriptions.
Provide accurate drug information to healthcare personnel. Monitor and maintain records on quantity, quality and type of drugs utilized.
Schedule/Shift: Full Time - 40.0 HRS, flexible schedule. Weekends, holidays and recess also required.
Required Skills and Abilities
1. Complete understanding and application of principles, practices, and standards in weight management care. Exceptional understanding of benefit design and management.
2. Current knowledge of health care trends and "at risk" population management principles. Exceptional interpersonal skills and ability to work both as a team member and leader.
3. Outstanding communication and presentation skills; both verbal and written.
4. Strategic thinker with proven ability to drive results. Strong analytical skills and ability to work independently in a project-based environment.
5. Intermediate computer skills required (Windows, Word, Excel).
Preferred Skills and Abilities
1. Completion of a PGY1 with rotation experience in ambulatory care or previous experience in weight management, diabetes care, or other related metabolic conditions.
2. BCACP, CDCES, or other certificate programs offered by APhA or TOS.
3. Proven experience utilizing EPIC electronic health record system is preferred.
Principal Responsibilities
1. Prepares and dispenses medications, performs clinical screening including checking for proper drug prescribed, dosage, drug interaction and instructions to patients.
2. Coordinates and participates in disease management programs.
3. Researches and identifies new medications for use in the inpatient and outpatient areas.
4. Presents information to pharmacy management and medical staff as needed.
5. Updates the formulary and notifies patients as appropriate.
6. Consults with clinicians on dosage, medication interactions and other clinical issues. Counsels patients on correct usage of drugs and on possible drug interactions with other prescriptions.
7. Provides accurate drug information to healthcare personnel.
8. Ensures adherence to policies and procedures, legal and regulatory compliance including Board of Pharmacy, HIPAA, DEA, USP. Supports organizational and departmental efforts in seeking or maintaining accreditation including Joint Commission and other federal and state agencies related to clinical practice.
9. Monitors and maintains records on quantity, quality, and type of drugs utilized.
10. Ensures emergency carts, Acute Care cabinets, and other clinical areas are stocked with proper medication.
11. Works on various pharmacy related committees.
12. May perform other duties as assigned.
Required Education and Experience
PharmD or graduate pharmacy professional degree and three years of related work experience or an equivalent combination of education and experience.
Required License(s) or Certification(s)
State of Connecticut Pharmacist license or Connecticut License eligible.
Physical Requirements
Ability to stand and walk for extended periods.
